America's Health Rankings United Health Foundation reported in 2020 that 33.7 billion dollars was spent in 2017 to address preventable hospitalizations. It has been shown that older adults who are hospitalized are 60 times more likely to develop disability than those who are not hospitalized. Multi- component therapy focused on identifying contributing risk factors while proactively addressing chronic disease management can effectively lower re-hospitalization likelihood. This workshop is designed to improve the healthcare professional's understanding of the potentially preventable readmission alerts that plague patientfunctionality and quality of life. Validated tools utilized to assess hospitalization risk will be explored. Case study scenarios will challenge the participant to synthesize multiple clinical factors in making evidence-based decisions. Upon completion of this course, the participant will have a greater understanding of, and proficiency in, the aptitudes necessary for navigating the snares of hospitalization risk.
