Effective Rehabilitation and Discharge Across the Continuum of Care

The services available throughout the continuum of care are ever shifting based on regulation and reimbursement demands. As therapists, we engage with the patient until they move to the next level of care. However, if you have not worked in other levels of care, it is sometimes difficult toanticipate what this next level entails. In today's fast paced healthcare climate, clinicians are pivotal in transitioning patients throughout the continuum of care.

This course will help the individual therapist gain a deeper understanding of settings outside of their own as well as community resources that can support a patient after discharge from therapy. This workshop will look to transparently dispel myths about levels of care as well as openly discuss the challenges associated. Therapists will use a case-based approach to understand the patient's journey from onset to discharge, what they experience, and how we as clinicians can enhance this process. Therapists will leave this course empowered in explaining to patients the servicesavailable outside of their normal practice area.
